Adult Placement Services
Purpose
To help individuals and their families determine whether placement can be prevented or is the most desirable option. If placement is the chosen option, we assist with the process of locating a facility either an adult care home, or a nursing home that can meet the needs of the individual.
Eligibility
Any Caldwell County resident is eligible.
Special Information
The person for whom placement is being sought must be willing to enter a facility.
The physician must complete an FL-2 medical form indicating the level of care he/she recommends:
(1) Rest Home level care requires residential care; 24-hour supervision; personal care; and administration of medicines. The process may take anywhere from one day to one month (nursing home placements often take longer).
(2) ICF (intermediate level of nursing home care) requires at least eight hours of nursing care per day by a licensed nurse with the remaining time and services provided by experienced staff.
(3) SNF (skilled level of nursing home care) requires 24-hour-a-day nursing care.
