Community Alternatives Program (CAP)

Purpose

To enable persons to receive nursing services in their own home. These services include in-home aide, nursing, medical equipment and supplies, physical therapy, speech therapy, occupational therapy, and lifeline.

Eligibility

Those individuals who require skilled or intermediate-level nursing home care.

The client must be Medicaid eligible.

Special Information

CAP/DA is for disabled adults 18 years or older.

CAP/C is for children newborn to 18 years old.

CAP/MR-DD is for persons with mental retardation and/or developmental disabilities. Services provided through Foothills Area Authority (Mental Health).
