Immunizations or Vaccines are made to stimulate your immune system to produce antibodies to fight off disease. After getting vaccinated, you develop immunity to that disease. This makes vaccines a powerful medicine that prevents disease!
We recommend that everyone receives needed vaccines. If you have questions regarding vaccines please ask your doctor or call us at 828-426-8400. You may request a copy of your immunization record by filling out the form below. Complete the form and bring copy to the health department, fax (828) 426-8527 it or email it to our immunization coordinator.

IMPORTANT INFORMATION: If you request an immunization record and no immunizations have been given at the Caldwell County Health Department, you must contact the last high school you attended (via email or in person only) and request a copy of your immunization record. Caldwell County Schools now require a processing fee per document requested. (Fee does not apply to students currently enrolled in Caldwell County Schools). Your cost for vaccines depends on your insurance coverage, how many and what types of vaccines you receive at your visit. Unless you qualify for state-supplied vaccines, there is a charge for each vaccine plus a charge to administer the vaccine.
For a self-pay client, your charge will be the fee for each vaccine you receive. Those costs can be found in our Administration Page.
Beginning Sept. 1st, 2015 Caldwell County Health Department will require all clients presenting for service to provide proof of income. Income will be assessed annually unless there is a change in financial status. Clients will be asked at each visit if there has been a change in financial status/number in the household.
Clients who have met the income criteria for the following programs (i.e., Medicaid, WIC, Food and Nutrition Services (FNS) Program (Food Stamps) and/or the Temporary Assistance to Needy Families (TANF) Program) will only be required to show documentation of currently eligibility and no additional income screening will be necessary.
NO client will be refused services when presenting for care based on lack of documentation, however each client will be billed at 100% until proof of income and family size is provided to the agency.
Cost is $20 for routine tests required for employment, but there is no charge if the client is symptomatic for TB or has been determined to be a contact with someone with active TB. If you have a positive skin test, our Communicable Disease Nurse will offer counseling, chest X-ray, and other services needed. TB control services are provided at no cost to the client.
Test are administered everyday but Thursday and some days close to holidays. If you receive a TB skin test, you are required to return to the clinic 48-72 hours after the skin test is placed and be evaluated by the CD nurse.
If you have had a live vaccine (such as MMR or Chickenpox) within the last 28 days, we will be unable to give you a TB skin test. Doing so may result in a false negative.
