Foster Care Provider Agreement

  1. General:

    I understand that this animal is property of Caldwell County Animal Care Enforcement until dispositioned and agree to return the foster pet to Caldwell County Animal Care Enforcement upon verbal or written demand by Caldwell County Animal Care Enforcement staff. Failure to do so may result in legal action.

    I agree to random site visits from Animal Care Enforcement staff that may be conducted between 8 am and 5 pm Monday – Friday at the address listed on file. During these site visits, I would need to make the animal readily available for viewing as well as the area where the animal is being kept. The foster care provider may be asked to correct deficiencies if the staff member performing the site visit finds deficiencies. Animal Services has the right to remove the animal from the foster care provider’s home if necessary.  

    Suppose I am fostering an animal through a 72-hour stray hold, and an owner comes forward to reclaim the animal. In that case, I must return the pet promptly upon verbal or written notification from Caldwell County Animal Care Enforcement staff.

    I will notify Caldwell County Animal Care Enforcement of any change in address, name, or telephone number within 24 hours.

  2. Care:

    I agree to provide the pet with a safe home, proper care and attention, and humane treatment in accordance with all current state, county, and local laws and ordinances. The pet should always be supervised while outside the home, wear proper identification, and not be allowed to roam at large. If the pet is unaltered, it shall not be allowed within mating proximity to unaltered animals of the opposite sex.

    I agree to feed my foster animal twice daily with at least an 8-hour interval between feedings unless otherwise directed by shelter staff or with written documentation from a veterinarian. I will feed my foster pet species-specific food. I agree to allow my foster animal access to clean, potable water at all times unless otherwise directed.

    I agree to keep the foster pet’s area clean and sanitary. I have been educated by the animal services staff about proper disinfection techniques. I agree to clean and disinfect the area where my foster is being kept at least twice daily.  

    I agree to observe my foster pets’ behaviors daily and note any abnormalities or changes. I have received proper training from the Caldwell County Animal Care Enforcement staff. I have been provided with a handout that outlines the best ways to monitor an animal’s behavior and habits. If behavior problems arise, I agree to make all attempts to relieve the problem, including notifying Caldwell County Animal Care Enforcement of the problem to help seek possible solutions.

    I agree to provide my foster pet with daily positive interaction and enrichment to encourage good behaviors.  

    If I am fostering an animal with a medical condition requiring medications, I will keep all necessary logs and return those upon request from the Animal Services staff. I agree to notify the Animal Services staff if my pet’s condition is worsening or with any other questions or concerns.

    I agree to notify Caldwell County Animal Care Enforcement if a medical emergency arises with my foster pet. I have been given emergency contact numbers to contact a member of Animal Services. I understand that if I take an animal to a veterinary hospital without contacting a staff member of Caldwell County Animal Care Enforcement, then I will be responsible for the payment of any unauthorized treatments. It does not show legal ownership of the animal, and I will not seek repayment for these expenses from Caldwell County Animal Care Enforcement.   

    I hereby certify that I am physically able to care for this animal. I understand that if for any reason, I am no longer able to keep this animal during the foster period, I may notify Caldwell County Animal Care Enforcement for assistance with placing the animal. I understand that the shelter may not have a space readily available and may take several days to find placement. I will notify the shelter staff at least seven days prior to any planned trips or vacations so that proper placement can be secured. I will need to provide a minimum of 14 days’ notice if the placement is required over a holiday.

    I agree to communicate with Caldwell County Animal Care Enforcement staff in a timely manner and schedule/attend any necessary appointments for the pet (at Caldwell County Animal Care Enforcement or another veterinary clinic).  

    Animal Services may provide supplies needed to care for my pet if available. I must return these items upon the return of the animal(s). If the item becomes chewed or worn, I must notify Caldwell County Animal Care Enforcement on how to discard the item. 

  3. Additional:

    I understand that any items (including but not limited to bowls, collars, leashes, food, etc.) purchased for this animal while it is in my care and at my expense are my responsibility and will be considered as a donation to Caldwell County Animal Care Enforcement if given to us upon return of the animal. It does not in any way show legal ownership of the animal, and I will not seek repayment for these expenses from Caldwell County Animal Care Enforcement.

    I agree to comply with all state statutes and local ordinances governing the licensing and ownership of animals. If unfamiliar with those statutes and/or ordinances, I will familiarize myself with them.

    If I decide to adopt this pet myself, I must notify Caldwell County Animal Care Enforcement staff by signing an "Adoption" contract and paying all necessary adoption fees within 48 hours of my decision. Notifying staff of my desire to adopt does not in any way guarantee that I will be able to adopt this animal, and I must adhere to all other policies and procedures about adoption from Caldwell County Animal Care Enforcement.

    I agree that I am at least 18 years of age and accept responsibility for the foster animals in my home through the foster-care program.

    I agree to notify Caldwell County Animal Care Enforcement of any other organizations I am fostering. I must have prior approval from the Caldwell County Animal Care Enforcement Director or designee to foster animals while fostering for another organization. Documentation for rabies vaccination status may be required.

    The foster care provider agrees to hold Caldwell County harmless and release the county from any claims, liabilities, and demands for restitution resulting from damage, loss, or injury that the foster care provider may sustain. Caldwell County, Animal Care Enforcement, does not warrant or guarantee anything about this animal, including health, physical condition, prior ownership, behavior, or disposition.

  4. Agreement

    I, the undersigned, do hereby declare that I, nor any persons living in my home, have ever been convicted of animal neglect, cruelty, abuse, or abandonment and will comply with the above requirements. I accept the animal as-is and assume all risk and financial responsibilities of fostering. I acknowledge that I have read, understood, and will comply with this contract’s conditions.
